Thank you for choosing TEAM ENDURANCE MMA.  We are very excited about the dynamic programs we have to offer, and we feel you will be as well.  The Summer Camp program is a fun, exciting and challenging learning environment centered on the arts of KICKBOXING & BRAZILIAN JIU-JITSU.  The Summer Camp program will give your child a structured environment that will challenge them both mentally and physically. 
Martial Arts offers a unique way to teach focus, discipline, confidence, respect for others, self-respect, commitment, concentration, peer pressure skills, ability to follow directions and proper etiquette.  All of the above are necessary tools for our children to cope with an ever-changing world.  We will provide a personal analysis for each new student attending: this will allow us to understand what you specifically want your child to accomplish in our Summer Camp and Leadership Program.

Tax Statements

Summer Camp is Tax deductible!  With Your records and our tax ID #, you will be able to file your deductions properly.   We do not disclose this information for any other purposes.  

Forms and Paperwork

TEAM ENDURANCE MMA must have all necessary forms completed; fees paid and on file before the student will be admitted.  This will ensure that there are no misunderstandings and that we have all necessary information to ensure your child’s safety.  Please keep all information up to date and notify us of any changes of address, phone numbers at home and work in writing as soon as they occur.  

Registration Fee: $50

All registration fees are due before the beginning of Summer Camp enrollment.  The registration fee secures your spot in our program.  We only have 10 available spots.  We cannot hold your position without paperwork and registration.  It is highly advisable that you purchase an additional uniform and t-shirt in the beginning.  It is very common that uniforms and t-shirts get lost and/or forgotten in the wash. This is a requirement and if a Martial Arts uniform is lost or stolen it must be replaced; we want you to fully understand what we specifically want your child to accomplish within our program.  WE ARE NOT A DAY CARE CENTER; we are a Professional Black Belt Leadership Academy that provides professional development to children and families. REGISTRATION FEES ARE NON-REFUNDABLE.

Tuition and Fees

Full Day 9A.M – 6P.M Monday thru Friday $199/Wkly

Half Day 9A.M-12P.M Monday thru Friday $159/Wkly  

TEAM ENDURANCE MMA operates on a budget, just like every other business.  We count on receiving payments on-time to ensure we satisfy our obligations and provide the quality instruction and service that our clients are accustomed to.  For that reason we have established a policy concerning payment and tuition fees. We require that your payment be made no later than the Friday before each week of Summer Camp.  
The fees for the Summer Camp Program are for each of the weeks of you have reserved, whether your child comes zero or all five days during the week, the weekly fee must be paid.  Should you wish to cancel a week you reserved for your child, a two-week written notice is required.  Should this notice not be given, you will be responsible for the weeks that you reserved.  Please notify one of Directors in the form of a written letter only.  We DO NOT pro-rate tuition.    
Note:  Multiple Family Parents that are sharing the cost of Summer Camp, we will not be accepting split payments for your child(ren).     

Late Pick-Up Fees

It is necessary for children to be picked up on time.  Parents who pick up their children after designated time will be charged a late fee of $2 per minute according to our clock.  Payments will be due at the time of pick-up.  


Returned Checks

There will be a $30 charge for all returned checks.  Returned checks must be paid with NSF fees within 7 days of the insufficient draft.  If payment is not received within 7 days, your spot will be forfeited and you will be responsible for any additional weeks reserved.  


Clothing and Personal Belongings

Each student will be solely responsible for his or her uniform (Gi), equipment and personal belongings. The Gi should is worn everyday Monday to Friday.  Girls should also bring a t-shirt (white, black or same color as the gi) or rash guard (under armor) to wear under their uniform and this should be part of their uniform.  In addition, girls with long hair need to have a ponytail band to pull their hair up during class.  
Students should keep personal items at home. Should a student bring personal items, TEAM ENDURANCE MMA is not responsible for these items.  Although electronics are allowed on a daily basis please be advised that there is a high risk of damage or loss.  We are not responsible for the items that the children bring.  In addition, all items should be personally marked with your child’s name.  
TEAM ENDURANCE MMA Summer Camp program has a temporary Lost & Found.  This is the best place to look for lost items.  Please label any and all clothing, backpacks, including Gi.  This enables lost items to be returned to their rightful owner.  Should you need to go through the lost and found, please take the time to put the items back in the bag, bin or box. 
Parents please remember that the mat (learning) area is off limits during daily activities for safety concerns and parents are asked to wait in the reception area for staff assistance before entering the training area.  Parents are however always welcome to watch the daily activities.  To do so all we need is proper notification.  


Pick Up From The Academy

When picking up your child from the martial arts academy please do not allow your child to walk from the building to your vehicle unattended.  We will not allow your child to walk to your car at the end of the day; they must be signed out and escorted by the adult picking them up.  Please notify the other adults on your pick-up list of our rules.  This if for your child’s safety and we are not negotiable at this request.  Should there be extenuating circumstances please call us on the phone and we will try to accommodate you.  TEAM ENDURANCE MMA is not responsible for custody arrangements, if there is a court order for any reason that effects our interaction with your child(ren), please bring us a copy of the court order and we will put it in your child(s) folder and will follow it to the letter.  Parents, we only allow adults 18 and over to pick up your child.

Illness

Your child’s health is of great importance.  If your child becomes ill while at the Summer Camp Program, you will be notified to pick them up.  This needs to be done within one hour.  Please make sure we have updated information on how to get in touch with you at all times.  No child may attend TEAM ENDURANCE MMA with a fever, any open or draining areas, including ears.  If any rash is present, you must speak with one of the senior staff members, a doctor’s note may be required before we allow them to attend.  We frown upon “sneaking” a child into camp sick.

   
Transportation

Not Offered At This Time.  Children must be dropped off to the academy by 9A.M 


Homework Assistance

Team Endurance MMA does NOT offer a Tutoring service, nor are we responsible for homework completion or school projects.  


Medication

A written statement from the parents and/or the physician must accompany all prescription medications. WE DO NOT ADMINISTER MEDICATION.   If your child cannot administer his/her own medication, a parent or qualified professional will be allowed to come and administer the medication for him/her.  


Emergency Drills

Emergency drills are held periodically to teach your child evacuation procedures.  We have exit plans located in the building to help the children remember the safest exit route in case of an emergency.   

Food

Parents, you need to pack a balanced and nutritional snack, please do not give children Mayo, Milk, Sweets or Soda; these are unacceptable.  Children feel better for martial arts class eating a sandwich, nutritional chips, fruit and plenty of water.  No canned energy drinks except Gatorade or Power Aid.  We would prefer flavored water to Gatorade or Power Aid, much less sugar.  Parents, if you forget to bring snack for your child, we will bill you $10 and snack will be provided as a one-time courtesy.  Please encourage you children to have their snack before class time.   

Injuries

TEAM ENDURANCE MMA makes every effort to prevent childhood accidents during their training times, but injuries are a normal part of growth and development.  Parents are responsible for medical bills that may arise from accidents.  We will notify you in the event of illness or accident and if we had to call for assistance from Emergency Services.  For this reason it is important to keep your records updated at all times, with current phone number. Please make sure that if your child is allergic to plants, animals, insects, food, or drink of any kind that this information is listed in the medical section. 
We do not immediately notify parents for minor injuries such as sprains, cuts and bruises.  We only will notify you if we feel medical attention is needed or has been notified.  We will, however, make you aware of any minor injuries upon pickup.     

Etiquette and Life Skills Training

Part of the Martial Arts training the children receive is etiquette training.  Due to this training, we have specific rules must be followed by students, families and guest.  Gentleman, please remove your hat when entering the academy.  Please do not walk on the training mat with your shoes.  Please do not chew gum in the studio.  Please do not bring outside food or drink in the studio.  When inviting or bringing guest, please inform them of the rules before they arrive so they are not offended or feel that they are being singled out.  All males are to remove any head cover or hat when entering the facility.  This does not include cosmetic covering such as a stocking cap.   

Martial Art Classes and Testing Days

We encourage our parents to be involved with the Academy and to support your child’s training.  We do require that unlike other team sports, there is absolutely no coaching from the side.  Please let the instructors do their job; encouragement from the side is welcomed, as long as it is not instruction on what to do.  Examples of positive encouragement are:  clapping, saying “good job” or “good work” and anything else along those guidelines.  Keep in mind that your child is not the only one on the mat and we need to keep the class environment conducive to the entire class’s well being.  
We are teaching these children life skills necessary to deal with a bully or aggressive individuals diplomatically, but we also teach them to defend themselves should the diplomatic process fail.  This is a contact sport and sometimes it is difficult to watch.  If something is difficult for you to spectate please get up and step outside or stay in the lobby.  Children feed off of their parent’s emotions and it sometimes causes a chain reaction.  If you are uncomfortable with something please come talk with us, we will try to help you through the emotion or we can remove your child from that particular drill or exercise.  
The children test every 3 months.  Each student is tested on their individual ability based around a school wide curriculum.  Please try not to compare your child to someone else; each child’s ability is not the same.  During the test please turn off your phones or pagers.  Please step outside to have any kind of conversation.  Do not bring laptops or handheld computers or electronic games.  It means a great deal to the children that they have your undivided attention as well as no distractions from the sound of a mobile phone, laptop computer or electronic game.

PARENTS!!!WE ARE A PROFESSIONAL BLACK BELT LEADERSHIP ACADEMY SPECIALIZING IN CHARACTER DEVELOPMENT.  WE ARE NOT A DAY CARE CENTER.  WE GO THROUGH GREAT PAINS TO DIFFERENTIATE OURSELVES FROM THIS.  PLEASE DO NOT REFER TO US AS A DAY CARE, AS WE ARE NOT.  WE ARE A MARTIAL ARTS AFTER-SCHOOL PROGRAM.  EACH CHILD IS REQUIRED TO PARTICIPATE IN A CHARACTER ANALYSIS INTERVIEW WITH (PROGRAM DIRECTOR DAVID ESCOBAR).  THIS WILL ALLOW US TO FOCUS ON YOUR CHILD’S MARTIAL ARTS DEVELOPMENT TO ENSURE A PRODUCTIVE SUMMER AND FOLLOWING SCHOOL YEAR.
